As a patient you are entitled to request access to view your medical record or have a copy as a electronic PDF. To do this, please fill in this form and email or hand it in at reception. We will require to see photo ID in person prior to releasing the request and the turnaround is up to 30 days.
Requests for under 18s - the parent or responsible adult is entitled to request - although those aged 12-18 where appropriate consent will be checked with the child first as 12's and above are deemed a legal adult.
If requesting for another patient - if the patient is deemed to have capacity we will require written consent from the patient to do this, if they do not have capacity then appropriate consent, e.g power of attorney documentation will be required to be checked.
For deceased patients - we would need to see proof of estate or documentation to show you are executor of the will to be able to consider this request.
